Steps for submittal:

Step 1 - Complete the required submittal form: Submittal Form

Step 2 - Name your image files like this: last name of the owner followed by the name of the patient followed by a number. For example, if owner’s last name is Jones and the patient’s name is Spot I would name my images Jones Spot-1.jpg, Jones Spot-2.jpg

Step 3 - Upload your images below and repeat the upload process until you have sent all of your images.

If your diagnostic imaging equipment is capable of sending images directly to another DICOM system via the internet please send them to:
